Abstract
초음파는 의학계의 여러 영역에서 다양하게 사용되어져 왔다1,2). 초음파를 이용한 골절단은 비교적 쉽게 이루어질 수 있으며 치조골을 절단함에 있어 그 치유에 해를 끼치지 않는다고 보고되었다3). Tomaso Vercellotti 교수에 의해 개발된 Piezosurgery는 이러한 초음파를 이용한 골절단술과 골성형술이 가능한 기구이다.
이러한 술식은 미세한 외상으로 인하여 지금까지 시행했던 것보다 매우 정확하게 골을 절단할 수 있도록 해 준다. 기구의 끝 부분은 조절이 가능한 초음파 주파수와 함께 흔들린다. 경조직과 연조직은 주파수에 따라 완전하게 절단할 수 있으며, 기구는 단지 단단한 물질만을 절단한다. 예를 들면 상악동 점막이나 신경 섬유 등과 같이 해부학으로 다루기 힘든 연조직을 보호할 수 있다(selective cut). 기존의 기구와 비교할 때 냉각제가 진동에 의해 기구의 끝부분을 따라 나와 깊은 부위에서도 효과적으로 냉각시킨다. 그 결과로 악골의 절단, 상악동 거상술, 입자형이나 bone block의 자가골을 채취하는 등과 같은 술식이 더욱 간단하고 안전하게 시행할 수 있다.
본 논문에서는 piezosuregy를 이용하여 시행된 이부골 이식술, 골창 형성술, 상악동 거상술 등에 대해 소개하고자 한다.
Piezosurgery uses modulated ultrasonic vibration to allow controlled cutting of bony structures. Delicate bony structures can be cut easily and with great precision without destruction of soft tissue.
Piezosurgery can be used to harvest the chin bone for intra-oral augmentation. Maxillary sinus membrane perforation is the most common complication that occurs with the sinus elevation augmentation procedure. This complication can occur during the osteotomy, which is performed with burs, or during the elevation of the membrane using manual elevators.
The purpose of this study is to introduce new surgical technique that simplifies chin bone graft and maxillary sinus surgery, thus avoiding perforating the membrane.
